Provides proof of health insurance coverage for an employee for income tax filing purposes. This report contains sensitive data and its use is restricted.

This report is produced annually (calendar year) and is available as a hard copy.

This report is also available to the Agency through RPCT Reporting Center and to the employee through EPP Employee Personal Page.
