Brief Description

Issued either to transfer employee leave balances to another Department or to certify the leave balances when an employee separates from Government service. This form contains sensitive data and its use is restricted.

This form is produced each pay period and is available as a hard copy or by electronic transmission. It is distributed by POI personnel office identifier.
