Provides a list of employees on leave without pay (LWOP) for at least 3 months due to a job-related injury whose Federal Employees Health Benefits (FEHB) carrier is not being paid through the automated system. The report advises the Agency to contact the Office of Workers' Compensation Program (OWCP) or the employee to determine if the FEHB Federal Employees Health Benefits premiums are being paid by OWCP Office of Workers' Compensation Programs. If the FEHB Federal Employees Health Benefits premiums are being paid by OWCP Office of Workers' Compensation Programs, the Agency should process Form SF Standard Form 2810, Notice of Change in Health Benefits Enrollment, through the automated system to remove the employee's FEHB Federal Employees Health Benefits enrollment code from the PPS Payroll/Personnel System database. The health benefits carrier will automatically be notified when the SF Standard Form 2810 is processed. Also, the Agency should notify OWCP Office of Workers' Compensation Programs of this change by mailing them all official personnel folder (OPF) copies of FEHB Federal Employees Health Benefits documents, including the SF Standard Form 2809, Health Benefits Registration Form, and the SF Standard Form 2810. The report is sorted by Social Security number. The report contains sensitive personnel data and its use is restricted.
