Maintaining Forms Records

The Forms tab on the CLER Federal Employees Health Benefits, Centralized Enrollment Clearinghouse System Main Menu provides the 2809 and 2810 options. Theses options contain the same data fields as Forms SF 2809, Health Benefits Election Form, and SF 2810, Notice of Change in Health Benefits Enrollment. With proper access authority, additional records may be added and current records may be updated or deleted.
