RFQS Election Form
Agency personnel can complete Form AD-961 to update distribution of the SF 50-B, SF 1150, tickler reports, and/or forms through RFQS.
To request a change by mail, return the AD-961 to:

To fax a change request, send the AD-961 to the Personnel Processing Branch at FAX Number .
