Maintaining Forms Records

The Forms option provides the Standard Form (SF) 2809and SF 2810 options. These options contain the same data fields as the SF 2809, Health Benefits Election Form, and the SF 2810, Notice of Change in Health Benefits Enrollment. With proper access authority, additional records may be added and current records may be updated or deleted.
